Step 1. Review Plan Details. To download a Schedule of Benefits for each plan, click on the Plan Details. Make sure the plan provides the coverage you seek and keep a copy for your records.

*These plans are scheduled reimbursement plans that base payment on the maximum reimbursed amount for each procedure. For actual amounts, please refer to the Plan Details.

Step 2. Download our brochure. Our brochure includes rates and an application. Rates listed do not include the $1 monthly Association Membership Fee, $5 Billing Fee, and a $10 one-time non-refundable Initial Enrollment Fee.

Step 3. Complete the application and mail it in along with your first month's payment and any applicable fees. Payment can be made by personal check, credit card or debit.
